Appointment Information


We encourage the continuation of existing physician-patient relationships and urge patients to consult with their primary care physicians before scheduling an appointment with us. In addition, some insurances require a prior authorization from a patient’s primary care physician before treatment by a nephrologist may begin. Although we will be happy to assist you in obtaining your referral, please be aware that it is the patient’s ultimate responsibility to obtain the proper authorizations and referrals.

After conferring with your primary care physician, his or her office staff may call us and set up your appointment or you may call one of our offices directly to schedule your appointment.
